ED Triage Program
In the ED Triage program, the Case Manager will provide transportation for patients being discharged from the hospital and for individuals traveling to and from medical appointments. Our goal is to meet the needs of each person receiving medical attention. At the beginning of each scheduled shift, the Case Manager will assemble food bags, hygiene products, and blankets which will be given to clients when they are returned to their homes. We will stop at the pharmacy, as needed, review the discharge plans for follow up medical appointments, and conduct an environmental assessment to assure that the client is being left in a stable and safe situation. Client information will be given to the Social Work staff who will follow-up with the client within 24-48 hours to address any additional needs that have been identified.
